                    APPLICATION,ITEM DOCUMENTATION 
                    REQUIREMENTS

    (a)  If an item other than that cited in the Procurement Identification 
Description (PID) is offered under provision 52.217-9002, (Sec L), of this 
solicitation, this provision specifies the Government's requirements for 
additional documentation needed to evaluate whether the offered item meets 
the requirements for the Critical Application Item (CAI) and/or Flight 
Safety Critical Application Part (FSCAP) identified in the PID.  The 
guidance herein is also intended to assist offerors in determining what 
documentation is needed to insure an adequate and timely evaluation of the 
source manufacturing the item.  Namely, a source approval request (SAR).  
The offeror shall determine which category applies.  The specified 
documentation for that category, as well as that specified for all 
categories at subparagraph (b), shall be submitted in support of the 
manufacturing process.

         (1)  CATEGORY I - Manufacturer of the same item for the Original 
Equipment Manufacturer (OEM) or for the Department of Defense (DoD). 

         (2)  CATEGORY II - Manufacturer of a similar item for the OEM 
or DoD.  (A similar item is defined as an item whose design, application, 
operating parameters, material, and manufacturing processes are similar 
to those of the item for which source approval is sought.)

         (3)  CATEGORY III - New manufacturer.  The exact or similar 
item has not been previously provided to the OEM or DoD.

         (4)  CATEGORY IV - Reverse Engineering.  Alternate manufacturing 
source when technical data is unavailable.  Sample parts may be provided 
to be used for design replication.  (Reverse Engineering is defined as a 
legal and ethical method of design replication by the development of 
unrestricted technical data, adequate for competitive procurement, 
through engineering evaluations of existing hardware.)

    (b)  Requirements for all CATEGORIES.

         (1)  Brochure(s) or synopsis of the company's capabilities, 
if not previously provided on other item(s) within the past year for the 
same weapons system, (e.g. F-16, C-5, H-60 etc).  Documentation shall also 
state if the company seeking approval is a nonmanufacturing source or the 
actual manufacturer.  If the company seeking approval is a nonmanufacturing 
source, the required information shall also be submitted on the manufacturer.

         (2)  A complete copy of process/operation sheets, including detailed 
sketches for the part, if Category I, or similar part, if Category II.  A 
description of all standard shop practices, such as cleaning, shall also 
be provided.

         (3)  Names, addresses, and telephone numbers of all subcontractors 
and/or vendors to be utilized for forging, casting, exotic material, and any 
special process such as finishing, heat treating, inspecting etc.  Special 
processes are those manufacturing processes that produce critical characteristics 
that cannot be verified after manufacture by non-destructive testing/inspection.  
Written evidence of the process approval from the prime contractor shall be provided.  
If a subvendor not currently approved by the prime contractor will be utilized, 
provide complete documentation substantiating the capabilities and qualifications 
of that subvendor.  It should be noted, however, that additional approval testing 
may be required.

         (4)  A complete description of current quality program (e.g. ISO, ANSI, 
OEM RATING) and a copy of the company Quality Assurance Manual, if not already 
provided on another request within the last year for the same weapons system.  
In addition, provide a copy of the latest survey results performed by a government 
agency and/or prime contractor, including pre-award surveys.

         (5)  A copy of the licensee agreement between the manufacturer and the 
OEM if such an agreement exists.

         (6)  Identify any "value added" provided by the prime contractor in the 
manufacture of the item.  "Value added" is any action, manufacturing or inspection 
process, data, instructions, or equipment that is essential to the manufacture of 
the item, but is not documented in the data package.

         (7)  If the SAR is for manufacture of Engine Spare Parts, a copy of the 
Requirements Control Card (RCC) shall be included with the purchase order(s) from 
the engine OEM.

         (8)  Any SAR identified to Boeing Rights Guard must comply with the Boeing 
Rights Guard Agreement. 

    (c)  Requirements for CATEGORY I.

         (1)  Current configuration drawings, with test procedures and top sheet 
of all applicable specifications and standards. 

         (2)  Copies of most recent purchase order(s), shipping documents to OEM, 
or signed DD Form 250 if shipped to DoD.  Documents must reflect part revision 
level.

         (3)  Summary of quality deficiencies experienced in the past three years 
during manufacture.  Include data relative to subvendors' actions and resolutions. 

         (4)  Copy of inspection method sheets used in manufacturing and final 
inspection.

    (d)  Requirements for CATEGORY II.

         (1)  A complete set of current configuration drawings and top sheet of all 
specifications and standards for the part for which approval is sought and for the 
similar item which the offeror is currently approved.

         (2)  Copies of most recent purchase orders or shipping documents to the OEM, 
or signed DD Form 250 if shipped to DoD, for the similar item. 

         (3)  A detailed comparative analysis of the differences and/or similarities 
between the similar item and the item specified in the solicitation; for example, 
materials, configuration, tolerances, processes, etc.

         (4)  A summary of quality deficiencies experienced within the past three 
years related to the manufacturing of the similar item, including data relative to 
subcontractor/vendors' actions and resolutions.

         (5)  A copy of all inspection method sheets used in the manufacturing 
and final inspection of the similar item.  Data shall include the actual tolerances, 
inspection devices, level of inspection, and resources performing the inspection. 

    (e)  Requirements for CATEGORY III.

         (1)  A complete set of current configuration drawings for the item.  
The top sheet of all applicable specifications and standards utilized in the 
manufacture of the item.

         (2)  A complete description of acceptance/inspection procedures used in 
the manufacturing of the item.  A list of any independent test labs the company 
intends to utilize in the manufacture of the item.

         (3)  A technical briefing may be required to assure the government of the 
firms' ability to manufacture the item.  A statement that the contractor is willing 
to provide such a briefing is required.

         (4)  Samples of the offered item(s) may be required.  (A statement on 
ability to supply samples is required.)

    (f)  Requirements for CATEGORY IV.

         (1)  All new configuration parts shall require provisioning drawings.  
These drawings may be identified with proprietary markings.  For repairable 
parts/assemblies, complete technical data (all component, specifications, and 
complete parts list breakdown) necessary for provisioning.  After initial 
review the Government will provide a detailed list of logistics requirements 
and any additional technical data required to support the new configuration. 

         (2)  A complete detailed manufacturing plan for the part including 
level III production drawings, process, materials, configuration, tolerances, 
testing, part function, and overall dimensions.

         (3)  The SAR shall identify if sample parts are required.  The 
Government reserves the right to provide/not provide sample parts based on 
availability within the Government supply system.

         (4)  Proof of FAA/PMA Endorsement indicating OEM equivalency.

         (5)  A technical briefing may be required to assure the Government 
of the firm's ability to manufacture the item.  A statement that the contractor 
is willing to provide such a briefing is required.  Sample(s) of previously 
reverse engineered parts, with the same complexity, may also be required.  
(A statement on ability to supply samples is required.)

    (g)  The offeror shall check below the CATEGORY that applies and include 
this part of the provision, as well as the additional required documentation, 
in support of the source approval request.
